What is the Hymen?

The hymen still maintains its social and cultural importance in our country and in many countries of the world. Anatomically, the hymen is a semi-flexible, thin structure that is located at the entrance of the vagina approximately 1.5-2 cm inside when you part the inner lips (small lips) of the genital area, with a small hole in the middle for the menstrual blood and vaginal secretions to flow, and which does not completely close the vagina. It is not like a curtain or tulle as it is thought, it is a fleshy structure that surrounds the entrance of the vagina, which can be of different thickness and shape.

What Does the Hymen Do?

The hymen completely covers the vagina in babies and children, thus preventing the contamination of feces from the anus into the vagina and entering the vagina in children who are tied up or who cannot maintain their personal hygiene on their own. Thanks to the hymen, microbes cannot infect the path leading to the uterus and ovaries, and it prevents the formation of urogenital infections, especially cystitis. From puberty, the hymen takes shape, stretches and expands to allow menstrual blood from the uterus to flow out. In Muslim societies such as our country, the hymen was evaluated differently and importance was given as the “first night bleeding-first sex partner” indicator.

Hymen Shapes

The hymen can be very thick, very thin or very flexible, with two or more holes in the middle. Sometimes the hole in the middle of the membrane is extremely wide and there is almost no membrane, in this case the penis can easily enter the vagina and the first night bleeding does not occur. In very rare cases, there is no hole in the membrane (imperforated hymen). In this case, menstrual blood can fill the entire uterus and vagina. This is a very serious condition and a small hole is made in the hymen in order not to damage the genital system of the woman, and the blood inside is evacuated.

The hymen is an anatomical structure that easily ruptures and bleeds when entered into the vagina with the penis, fingers or examination instruments. However, due to interpersonal structural differences, blood may not come at the first sexual intercourse in cases where the hymen is extremely flexible or the membrane structure contains thin and few vessels.

What is Hymen Stitching?

Due to cultural reasons, the importance of the hymen is still up-to-date, and for this reason, hymen planting is still in demand. Hymenoplasty (hymenoplasty)  or  hymenorrhaphy for hymen  repair is named. For this operation, it does not matter when or how many times the person has had sexual intercourse before. The hymen can be repaired if desired, even in women who have had an abortion or have given birth. In fact, it is not possible to completely restore the damaged membrane. However, a new hymen can be made with the pieces taken from the vaginal wall. Thus, in the first sexual intercourse, when the penis enters the vagina, this hymen is torn again and the desired hymen bleeding occurs. When the operation is decided, the width of the vagina, the type of hymen, the number of tears are evaluated in the examination and the operation technique is decided.

What are the Types of Hymen Planting?

Temporary hymen  implantation is generally done 25 days before the wedding or possible sexual intercourse. In temporary hymen suturing, the previously torn parts of the hymen are repaired and self-melting suture material (thread) is used to facilitate the union of the tissues. It can be done with local or general anesthesia. It can be done from 25 days before the wedding (whether 10 days or 2 days). The process takes about 20 minutes. If your stitches have not yet melted in the desired time, but the tissues have held your threads together, your doctor may take it 1-2 days before the intercourse, if necessary, so that the threads are not noticed.

If you do not have a wedding date in front of you and you want a hymen repair, “permanent hymen stitching” , that is, “permanent flap method”   , is done. In this operation, the remains of the hymen are combined with the flap method and the hymen is created again. It is preferred to be done with sedation anesthesia. It may take 30-45 minutes.

Is Permanent Hymen Repair Performed by Gynecological Examination?

Obstetricians and Gynecologists can understand the structure of the hymen, whether it is torn or not, and if it is torn, whether it is an old tear or a new one. If the hymen has been planted and the person has not had sexual intercourse yet, an obstetrician can see the findings of the operation in the hymen. However, in the examination performed months and years later, the findings of the previous surgery cannot be detected in a person who has previously had a hymen stitched and the first night rupture has occurred, that is, it is not understood whether he has had a hymenoplasty operation (hymen surgery) before.

Can Hymen Examination Be Performed Anywhere?

Hymen examination can be done in private practice and private hospitals, but an official letter is not issued. In addition, the patient does not have to report the result of the examination to anyone other than himself. If you are going to be examined in a state hospital and you want an official letter, you must first apply to the prosecutor’s office and get a petition.

Physicians are free to refuse to do or report on a hymen examination unless they are assigned to an official institution and are asked to do a hymen examination in an official letter.

Hymen Suturing with the Flap Method Technique

The most definite and pleasing one is the  “permanent flap  method” is The difference of this operation from other techniques is that it eliminates the necessity of performing 4-5 days before the wedding. Bleeding occurs when the patient experiences the first sexual intercourse after 3 months or 3 years after the procedure. It is not understood by the person to have the intercourse that the hymen is sewn, but it can be understood that the operation is performed by an obstetrician or forensic doctor. This operation is performed under sedation anesthesia. The operation time is 30-45 minutes. varies between After the procedure, the patient is rested for 1 hour, bleeding and pain are monitored, then the patient can easily walk, go home, travel. He should definitely apply the prescription recommended by his doctor and pay attention to genital hygiene. Since self-dissolving sutures are used in the operation, it is not necessary to remove the sutures afterwards.

What Should Be Considered After Hymen Planting?

Hymen suturing is performed under local or sedation anesthesia, depending on the technique applied. After the operation, you can walk home or go on a journey. If you have received sedation anesthesia, you may need to rest for 1-2 hours after the operation. In order to preserve the strength of your stitches, you are asked not to take a hot shower, not to go to the Turkish bath or sauna, not to lift heavy loads, not to be constipated, not to do sports such as swimming or jogging for a while during the recovery period. It is recommended that you use the prescription given by your doctor (antibiotics and painkillers) and pay attention to your personal hygiene.
